- National Health Identification number (NHI), skin images that may identify you, skin medical history and general health information relevant to skin cancer screening and management of potential skin cancers.
We collect your personal information in order to:
- Correctly identify clients, screen for skin cancer, accurately assess skin cancer risk, allow monitoring of skin lesions with digital images, have reliable ways of communication and follow-up with clients, coordinate appropriate treatment of skin cancers with client and 3rd parties directly involved in client’s treatment, and share appropriate information with client’s GP and other health providers directly involved, for continuity of care.
Besides our staff, we share this information with:
- Client’s GP, the client, and health service providers that are directly linked to care such as specialists, and laboratory services in order to provide timely, accurate screening, management of risk and treatment of skin lesions.
Providing some information is optional. If you choose not to enter a certain type of personal information, we may not be able to provide a skin screening service at all, or at least limit the accuracy of the assessment or ability to provide the best care for skin cancer screening, surveillance or management.
We keep your information safe by storing it at a high level of encryption of files in the cloud, not onsite, and only allow staff access to information that they require to do their tasks.
